After completing your form, please email to dni@destinywo.com. Once received and approved, you will receive an email link to make the payment for the application fee. If you prefer to mail in your $100.00 application fee you may send payment to: Destiny Network International, 7700 N. Council Rd, Oklahoma City, OK 73132. (Applications will not be processed until your application fee is received)
